Learning labs with practice staff and My Care My Way team - a proactive and holistic care model for over-65s, moving care closer to home and keeping people well
MCMW is a new way of working with people aged 65 and over. It brings together physical health, social care, mental health and the voluntary sector services to plan and deliver care with patients that meet their holistic needs. Between 2017 and 2018, Innovation Unit worked with West London CCG to help design and mobilise this new model and new ways of working. In summer 2018, we worked closely with the My Care My Way integrated teams to deliver learning lab sessions within 44 GP practices, using our newly designed practice diagnostic tool as a way to help the team understand where their services are on their journey, find areas of development and identify key opportunity areas. MY ROLE - I lead on facilitating the learning lab sessions within over 40 GP practices, as well as developing and designing the diagnostic methodology we used for the sessions. Throughout the process, I developed the session plans, directed the design process and engaged GPs and My Care My Way teams.
